RELEASE OF MILITARY RECORDS
The National Archives and Records Administration (NARA) has outlined certain requirements for the dissemination of military records, such as the DD Form 214, which this office has elected to adopt. Attached is a DD Form 214 Request Form that must be filled out by any individual requesting a copy of a DD Form 214 from this office. ALL requests for DD Form 214 must now be accompanied by this written request form and a picture I.D. Copies will only be distributed to the veteran listed, or to his or her next of kin. Lawyers, doctors, historians, etc., may request copies of a DD Form 214 with signed authorization from the veteran or next of kin.
For your convenience, the DD Form 214 Request Form is provided in Adobe Acrobat format, which requires you to have the free Adobe Acrobat Reader installed on your system for viewing. The form may be printed out and completed prior to your visit to the Recording Department's downtown location.
